IMPORTANT: The following is also a list of additional documentation we’ll need from you in order to work on your SFLTC claim. ALL documents have to be submitted before we can process your claim. The sooner you upload the required documents, the faster we can lock in your refund.

A copy of your 2019 tax return (including all pages and schedule C)
A copy of your 2020 tax return (including all pages and schedule C)
A copy of your 2021 tax return (including all pages and schedule C)
A copy of your valid drivers license
