Required Documentation

IMPORTANT: The following is also a list of additional documentation we’ll need from you in order to work on your SFLTC claim. ALL documents have to be submitted before we can process your claim. The sooner you upload the required documents, the faster we can lock in your refund.

If you have any questions or need additional help uploading your documents, please feel free to give us a call at 480-550-9599.

Please upload the following documents below:

  • A copy of your 2019 tax return (including all pages and schedule C)
  • A copy of your 2020 tax return (including all pages and schedule C)
  • A copy of your 2021 tax return (including all pages and schedule C)
  • A copy of your valid drivers license

UPLOAD INSTRUCTIONS:

  1. Click on the "browse" button next to each document request below.
  2. Locate the document on your computer and select it.
  3. NOTE: (Once the document is finished uploading you will see it's file name listed).

IMPORTANT: Do not click the "Submit Documents and Continue" button until you have browsed for and selected all of your documents. Once you've attached all of the documents click the blue "SUBMIT DOCUMENTS AND CONTINUE" button.
